A joint enterprise between NHS and social services.
It provides a home-based service for school-age children with disabilities.
The team includes: clinical psychologist, community nurse, health visitor, paediatrician, psychiatrist, social worker, support worker, secretary.
The team provides: psychological support, help managing difficult behaviours, arranges and co-ordinates specialist services, encourages intergration, counselling, teaching/developing self-help skills (continence, personal hygiene, dressing, feeding), arranging respite care, information about disability services.
They also provide help to professional colleagues and voluntary agencies.
Referrals usually come from school doctors, teachers, area team social workers, GPs, Child Development Centre (eg at Addenbrooke's), health visitors. Parents can contact them directly and their consent is required before a referral is made.
