Other or former names: Welcome Home from Hospital
'Welcome Home from Hospital' has combined with 'Help At Home' to become one service known as 'Help At Home'.
Anyone can refer a patient.
The scheme organises volunteers to help patients discharged from hospital who need short term (up to two weeks) help to settle back home. The volunteers help people in a variety of ways: shopping, light housework, checking to see if someone's okay etc.
The service is free and confidential, and is provided by trained volunteers, who all wear identification badges. Written consent from the patient (or their carer) will be obtained before any details about the patient are passed on to a volunteer.
